All student-athletes 6-12 must have the following documents completed and on file with the athletic office prior to the first practice.  Forms are available in the office, as well as, available here in digital format.


As of June 27, 2016 the IHSAA has expanded the number of medical professionals who can sign a physical form.

“Effective immediately, the IHSAA pre‐participation physical evaluation form may now be completed and signed by, not only a physician with an unlimited license to practice medicine, but also nurse practitioners and physician assistants clearing high school students for athletic participation.”

In addition to the physical form, there is also information for parents and students about sudden cardiac arrest and concussion.  There is a consent form that needs to be signed by both a parent and the student.
